Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical and dental needs quickly upon arrival. 

Payment Options and Dental Insurance

For your convenience, we accept cash, personal checks, debit interac, e-transfer and most major credit cards. Payment for all services rendered is due at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

We encourage patients to familiarise themselves with their insurance. While it is not the responsibility of the clinic to track or monitor your insurance, out team will make every effort to help you. Ultimately, the goal of our dental health providers is helping you achieve oral health which does not always line up with insurance coverage.
